Request for Travel Outside The U.S.
Complete and submit this form along with your most recent I-20 to the International Student Program office no later than 7 days prior to departure.
Warning: If you decide to travel outside the U.S, please be prepared, understand the risks, and be advised that you must have valid documents in order to return. For travel updates please check with your country's embassy and the U.S. State Department website: https://travel.state.gov/content/travel/en/us-visas.html.
Your signed I-20 (travel signature on page 2) will be available for pick up 3 -5 business days.
Have physical copies of these items in your possession prior to leaving the U.S. as you may be required to show them at the port of entry upon return:
- I- 94 information (https://i94.cbp.dhs.gov)
- Current and valid F-1 visa (please see below if your visa has expired)
- Transcripts (Please note that some U.S. Consulates may require official transcripts)
- Proof of payment and registration for following school term
- San José City College Student Identification Card

Visa Revocation
If you have been arrested in the USA, please inform a DSO prior to travel as your F-1 visa may have been revoked and no longer valid for travel.
